Mechanical Stretching Devices for the Treatment of Joint Contractures of the Extremities

Monday, August 8th, 2011

Purpose of Technology

Mechanical stretching devices are used for the prevention and treatment of joint contractures of the extremities, with the goal to maintain or restore range of motion (ROM) to the joint. These devices are intended to replace or complement physical therapist-directed sessions by providing frequent and consistent joint mobilization under controlled conditions in a hospital setting or in the patient’s home.

Questions addressed in this report include:

  • Do mechanical stretching devices improve health outcomes in patients after joint surgery or injury when compared with conventional physical therapy?
  • Are mechanical stretching devices safe?
  • Have definitive patient selection criteria been established for mechanical stretching devices?

These questions provide a framework for review and synthesis of the best available evidence, which is presented in this report, along with the Hayes Ratings for the technology. This report also provides background information about the condition being treated, a description of the technology, and the context for development of the technology. Other key elements addressed in the report include the regulatory status; applicable Medicare and commercial payer coverage policies; relevant ICD-9 and ICD-10 diagnosis codes; cost and cost effectiveness; credentialing; hospital length of stay; need for large case management; and ongoing and future clinical trials of the technology.
